MDS1 is located approximately 300 kb upstream of EVI1. It can be expressed as a separate transcript that codes for a protein of unknown function, but it can also form part of an alternative EVI1 mRNA in normal tissue [PMID: 8643684]. Translation of the MDS1-EVI1 mRNA gives rise to a protein that turns the transcriptional repressor EVI1 into an activator [PMID: 9067573].

A chromosomal aberration involving MDS1 is found in a form of acute myeloid leukemia (AML) [PMID: 8643684].
